Year 12 - How do I apply?

All students

Please provide:

1. A copy of your most recent, complete & official* semester report card (i.e. Year 11, Semester 1), and
2.  A completed permission form (below)
3. A passport and visa document (International students only)

*Not TrackEd document. Your full, school-issued report card is required.

Please download the Permission and Declaration Form, and read and complete all fields.

Firstly, seek parental/guardian consent, then seek permission from your school. 

You may need to contact your school to find who is the best person to provide this consent (e.g. Guidance Officer or Principal).

Ensure the form is fully completed and signed by yourself, your parent/guardian and your school.
